Additional COVID-19 Clinics Scheduled for Children Ages 5-11
FOR IMMEDIATE RELEASE: 12/2/2021 LICKING COUNTY, OHIO –
The Licking County Health Department (LCHD) will offer COVID-19 vaccinations for children ages 5 to 11 during two additional community clinics that are scheduled in different locations in the county. To schedule an appointment for a clinic CLICK HERE.
The clinics will be held at the following locations:
Wednesday - December 15 Thursday - December 16 Monroe Township Fire Station Hebron Municipal Complex 186 East Coshocton Street 934 West Main Street Johnstown, Ohio 43031 Hebron, Ohio 43025 3:00PM - 5:00PM 3:00PM - 5:00PM
Parents/Guardians will need to make an appointment for their child using the link above for these clinics. Only children ages of 5 to 11 will be vaccinated during these clinics.
All children under 18 must be accompanied by a parent or guardian to receive a vaccine. If a minor arrives with an adult who is not their parent or guardian, they must have a written statement from the parent or guardian granting permission for the child to receive the vaccine.
